After 6 years Rosy Abate leaves the prison for good conduct and is granted by the court the custody of Leonardo, who is now 17 years old and is in Naples and to live works as a mechanic and as a waiter. Regina still tries to insinuate in Rosy the doubt that Leonardo is not her son. Leonardo, involved in a brawl to defend a girl, Nina Costello, daughter of a camorrista, from an attempted violence, is stopped by the police and then released thanks to the intervention of Luca. Leonardo's troubles are not over: threatened by a drug dealer, called "il Mastino", with whom he has a debt of 20000 euros, he turns to Don Costello for a loan to settle the slope. Don Costello then has both Mastino and Nadia, Luca's companion, killed, causing Leonardo to be blamed for killing the woman. Rosy then makes her son flee, now become a criminal, to protect him from arrest.
Luca, now convinced that Leonardo is guilty of Nadia's murder, confronts Rosy, who tries to defend his son on the grounds of lack of evidence. Regina advises Rosy to take responsibility for the murder in the place of her son and to set herself up, but Costello has a fake lawyer intervene who, with a technicality, cancels Rosy's arrest just as she is about to make his deposition to the police. Weaving a long and patient plot, Costello eventually leads Rosy to join his family in exchange for Leonardo's promise to stop taking drugs. Antonio confides to Rosy his plan to leave the Holy Alliance to manage his traffic independently and less violently, obtaining the support of women. Actually Rosy tries to find a way to frame the boss and set up a trap to have him arrested but an anonymous phone call makes the blitz fail. Rosy then tries to convince her son to leave with her for Turkey and start a new life in this way, but Leonardo refuses to reproach them for having betrayed Costello, to whom he is now faithful. It turns out that the author of the anonymous phone call that saved Costello is Commissioner Gagliardi, Luca's boss, who also reveals to Rosello the double game of Costello and commits to eliminating both her and Luca.
Luca manages to put a killer sent by Costello to kill Rosy. Ciro Gargiulo, Leonardo's friend, dies in a firefight between Costello's men and those of the Baron. During an interview at the police station, Bonaccorso punches Costello with a punch, thus giving Gagliardi the pretext to lift him from the investigation. Rosy tries in vain to convince Costello to leave Leonardo free to make his life and so decides to enter the Holy Alliance to take revenge. During Nina's eighteenth birthday party, Rosy tries to kill Costello but he anticipates her, injuring her. Rosy escapes and Leonardo, to protect her, hides her and tells that she committed suicide by throwing herself into the sea. Meanwhile, Costello's men eliminate all the members of the Holy Allenza; only the Baron manages to save himself by putting himself on the run. Luca comes to discover that Leonardo is the son of policeman Ivan Di Meo. After discovering that Rosy is still alive, Costello tells Leonardo to kill his mother in the presence of his men but Leonardo manages to make her flee by making her a shield. Costello then has Leonardo arrested by Gagliardi's men. Meanwhile, from the video recordings, it turns out that Nadia's real murderer is not Leonardo but Costello himself.
The intervention of the authorities with the consequent arrest of Leo throws both Rosy and Regina in complete despair. The young man is taken to the prison of Nisida, becoming a cellmate of some "pawn" boys of Antonio Costello. By the intercession of the quaestor Licata, it is ordered that Leonardo be transferred to a safer prison but, at the request of Costello, Leonardo is beaten up, managing to get by with broken ribs and some bruises. Thus the fear of Rosy that the prison could become Leonardo's tomb grows more and more: the ex-Queen of Palermo convinces both Regina and Luca to collaborate to save the boy making him escape. Luca agrees to establish together with them an unprecedented action team, now sure of Leo's innocence in Nadia's murder. Once the plan was successful, Rosy, Leo and Regina are ready to leave Italy and start a new life; in fact, thanks to Luca's help (which meanwhile has to deal with his colleagues, all corrupted by Costello) they manage to get passports. Leonardo, however, having overcome a terrible heroin withdrawal crisis, does not intend to leave without saying goodbye to Nina: so he pleads with Regina to go to her, and to greet her on her part. After an initial reluctance, the night before departure, Mainetti went to Nina, who revealed that she possessed evidence of Leonardo's innocence: having returned from Rosy and Leo, the girl revealed that she had discovered spying on a discussion of her father, that Diego has the video of the surveillance cameras of the shed where the inspector Nadia Aversa was murdered (by Costello and not by Leonardo). Suddenly Maometto and other Costello's men who had followed Nina burst in: a terrible firefight broke out in which Queen and Mohammed lost their lives, shot at each other. Rosy, Luca and Leo go looking for Diego, but they arrive too late: the boy has already dealt with Costello who had him assassinated. On his deathbed, he reveals to Leonardo that now Antonio has the video.
