Während der Jagd auf einen Bankräuber erleidet Bosco plötzlich eine Panikattacke. Mit Symptomen wie Atemnot und Schmerzen in der Brust wird er ins Krankenhaus eingeliefert. Dort bescheinigt man ihm einen ausgezeichneten Gesundheitszustand, rät ihm aber, einen Psychologen aufzusuchen. Bosco befolgt den Rat und landet bei dem Therapeuten Brian O'Malley, der einst selbst Polizist war und Boscos Probleme aus eigener Erfahrung kennt. Obwohl O'Malley äußerst feinfühlig vorgeht, weigert Bosco sich jedoch strikt zu akzeptieren, dass seine Probleme auf ein Trauma zurückzuführen sind, welches er bei seinem Einsatz am 11. September erlitten hat. Als der entflohene Bankräuber gefasst wird, verliert Bosco die Beherrschung und schlägt dem Mann brutal ins Gesicht. Er wird vom Dienst suspendiert und sieht sich nun endlich in der Lage, über die verdrängten Erlebnisse am 11. September zu sprechen. Doch nicht seinem Psychologen, sondern seiner Partnerin Faith vertraut Bosco sich an. Carlos will sich um seine acht Monate alte Tochter Kylie kümmern, bis man geeignete Adoptiveltern gefunden hat. Nun hat er alle Mühe, seine Rolle als Vater mit seinem beruflichen Alltag zu vereinen - zumal sich das engagierte Kindermädchen als überforderter Teenager entpuppt. Um Carlos zu entlasten, passt sein Mitbewohner Davis auf das Baby auf. Als er es jedoch wickeln will, hat es hohes Fieber und schlimmen Ausschlag. Sofort fährt er mit Kylie ins Krankenhaus, doch schon unterwegs setzt die Atmung des Mädchens aus...

Bosco's emotional turmoil escalates when he begins to experience panic attacks and flashbacks, but continues to deny that there's anything wrong. After he has a panic attack while driving the patrol car and nearly causes a crash, Sully insists that Bosco see his friend Brian O'Malley, who's a therapist and a former cop. Bosco angrily resists O'Malley's suggestion that the panic attacks and flashbacks to the incidents with Shaquana Golden, Jared McKinley and Glen Hobart are the result of September 11th post-traumatic stress disorder, and storms out of the counseling session. His personal crisis comes to a head when he is suspended from duty for a week and ordered into mandatory counseling for punching a suspect already in custody. Shaken, he visits Faith at home and finally breaks down, relating the horror of watching people fall to their deaths from the Towers, unable to help them, ashamed that his fear left him frozen and unable to go back and help anyone else for several hours. Enfin tiré d'affaire, Davies est autorisé à regagner son domicile pour entamer sa convalescence. Ses relations avec Sully sont assez tendues depuis le drame, malgré les tentatives de celui-ci pour renouer leur amitié. Cette dissension est d'autant plus difficile à supporter pour Sully que son épouse, Tatiana, s'est enfuie avec son fils à la suite d'une énième dispute... Carlos reçoit la visite d'une jeune femme, un nourrisson dans les bras. Elle se présente comme la soeur de l'une de ses ex-petites amies, laquelle aurait eu un bébé né de leur liaison. L'inconnue le prie d'assumer son rôle de père en se chargeant de l'enfant, le temps de lui trouver une famille d'adoption...

Bosco si oppone alle idee di O'Malley che pensa che gli attacchi di panico e i flashback su Shaquana Golden, Jared McKinley e Glen Hobart sono i risultati post traumatici degli episodi delll'11 settembre. Viene sospeso dal servizio per una settimana per aver picchiato per la strada, alla presenza di testimoni, un sospettato già preso in custodia. Agitato e scosso, va a casa di Faith e alla fine crolla, raccontando l'orrore di aver visto persone buttarsi dalle Torri, impossibilitato ad aiutarle, si vergogna che la paura lo ha ghiacciato e non può tornare indietro per aiutarli. Carlos intraprende l'iter per un'adozione aperta per sua figlia Kylie, ma non si presenta ad un appuntamento con dei potenziali genitori adottivi perché si è molto affezionato alla bambina. Ty si offre di aiutarlo facendo da babysitter, e salva la vita di Kylie facendole la respirazione bocca a bocca quando, malata, all'improvviso smette di respirare. Fred progetta un futuro migliore per la sua famiglia, se Faith passa l'esame da sergente, e reagisce male quando Faith gli dice che potrebbe essere bocciata in quanto era preoccupata per Bosco.
