Unfallopfer Ryan Buckley befindet sich auf dem Weg der Besserung. Das Rückenmark war offenbar doch nicht so stark verletzt wie befürchtet, und der junge Mann wird bald wieder laufen können. Doc ist erleichtert, hat aber keine Sekunde Zeit zum Aufatmen: Carlos fällt wegen einer Gehirnerschütterung aus, und sein Dienst muss übernommen werden. Als Doc die Formalitäten erledigt, trifft er auf seinen ehemaligen Kollegen Jerry Mankowitz, der nach einer Schussverletzung aus dem Dienst ausgeschieden war. Bei einem gemeinsamen Essen erzählt Doc seinem alten Freund von seinem folgenschweren Fehler bei der Behandlung Ryan Buckleys, und Jerry rät ihm, sich von Buckley fern zu halten.
Im Krankenhaus findet Carlos unterdessen nur wenig Ruhe. Die Krankenschwestern sind schlecht auf ihn zu sprechen und haben sich deshalb eine besondere Gemeinheit für ihn ausgedacht: Er muss das Zimmer mit einem Alkoholiker teilen, der in seinem Entzug permanent von Wein- und Schreikrämpfen geschüttelt wird. Da an Schlaf gar nicht erst zu denken ist, spaziert Carlos in der Nacht über die Flure und sieht dabei Jerry Mankowitz im Gespräch mit Ryan Buckley. Wie Ryan behauptet, arbeitet Jerry für seinen Anwalt.
Auch Sully verbringt einen Großteil seines Arbeitstages im Krankenhaus. Seine Mutter Irene hat sich beim Kochen verbrannt, und Sully erkennt, dass es ein Fehler war, sie aus dem Heim nach Hause zu holen. Unterdessen begibt sich Bosco unter Missachtung aller Vorschriften auf Dealerjagd. Doch als er endlich einen Drogendealer dingfest macht, erkennt er in ihm seinen Bruder Michael...

Carlos decides to take a ride on the fire pole and falls, losing consciousness. Doc, Kim and Alex bring him to the ER where he is finally awake and combative. Dr. Thomas admits him to the hospital for observation because of his head lac and LOC. The nursing staff settles a score with Carlos by putting him in a room with Rodger, a drunk he brought in the day before. Rodger is alternating between screaming, crying, whimpering and non-stop talking -- all at the top of his lungs. He's going through the DTs and can't be sedated, because he admits to being drunk since 1987.
While Doc is in the ER filling out the paperwork on Carlos's trip to the hospital, he runs into his old partner Jerry Mankowitz. Jerry insists upon taking him out to dinner, during which Doc opens up to him about removing Ryan's cervical collar. He admits that he was jealous of all the attention that the documentary crew was paying to Carlos, and won

Rodger grida, piange, non smette mai di parlare e si lamenta - tutto a pieni polmon, ma non può essere sedato perché è alcolizzato dal 1987.
Mentre Doc è nel pronto soccorso per andare a trovare Carlos si imbatte nel suo vecchio partner Jerry Mankowitz. Jerry insiste per portarlo guori a cena durante la quale Doc gli confida la storia del collare cervicale di Ryan, e si domanda se è stato ripreso dalle telecamere del documentario che stavano girando quel giorno mentre toglieva il collare cervicale Jerry gli dice che ha fatto la cosa giusta per salvare la vita a Ryan. Gli dice anche che non deve più fargli visita in ospedale, potrebbe attirare l'attenzione tale interesse su un caso.
Doc lavora con Kim e Alex, con le quali si trova molto bene. Quando esce per una chiamata per soccorrere un uomo ferito durante una rissa in un bar trovano anche Sully e Ty, Kim nota che c'è qualcosa fra Alex e Ty. Quando Kim glielo chiede, Alex ammette che c'è qualcosa fra lei e Ty.
Quando Tatiana lascia la madre di Sully sola per qualche minuto per andare a comprare il latte, la signora cade dal letto e si rompe il braccio. Al pronto soccorso, il dottor Thomas dice a Sully che la ferita è seria e potrebbe richiedere un'operazione, che si potrebbe complicare a causa del diabete. La ricovera e dice a Sully che sua madre dovrebbe essere ricoverata in una casa di cura. Sully è fuori di se ma la sua ira nei confronti di Tatiana per aver lasciato la madre da sola si placa quando vede quanto è in pena e disperata per la ferita della suocera.
