The film begins with a boy named Michael, a kid who has a crush on a beautiful girl named Jada but is always rejected because Jada is stuck with a gangster, Lorenzo (William L. Johnson). When Michael comes close to her, he is beat up by Lorenzo and his homeboys, T-Bone and Baby Boy. But Michael is secretly involved in black magic, and tries to bring a doll that he calls Killjoy to life. After he is abducted by the gangsters and brought into the woods, Michael is accidentally shot and killed by Lorenzo.
The film begins with a young man named Nic (Charles Austin) being chased by two police officers (Wayland Geremy and Bobby Madsen) for selling cocaine. A week later, Nic, his partner-in-crime Ray-Ray (Choice Skinner) and three other delinquents: Eddie (Jermaine Cheeseborough), Ce-Ce (Nicole Pulliam) and the shy Charlotte Davis (Olimpia Fernandez) are taken by two detention officers, Denise Martinez and Harris Redding (Debbie Rochon and Logan Alexander) to Loxahatchee Canon, where there is a detention center for the delinquents to stay in order for them to pay for their crimes. On the way down, their engine blows in the middle of nowhere, with no cell phone reception, no gas stations nearby and no hope.
Some time passes and Killjoy is once again called, this time through a blood pact. Immediately he resorts to using the blood spilled by his summoner to create three underlings, which he dubs new evil clowns, ''Punchy'', ''Freakshow'' and ''Batty Boop''. However the man does not name a victim for Killjoy, leaving the scene without doing so. This causes Killjoy and his posse to vanish and return to their world.

But one of their tormented "patients" has returned for revenge. Now, The Gore Collector, and his mulitiversal traveling companions, Molly and Tape Head, have finally uncovered the whereabouts of the Psycho Surgeons and get set to exact the bloodiest of revenge. But what does this mean for The Gore Collector himself? As he returns to where it all started - the Bunker of Blood - he will find out what this splatter-soaked road trip across a fevered nightmarescape has REALLY done to his mind AND body!
